As a Service Coordinator on our EDN team, you will fill your passion bucket by supporting families and their children with developmental needs. To best support these families, you will be responsible for the identification, referral, and coordination of services their infants and toddlers with special needs require to support their development. You will coordinate these services by communicating with new clients, physician offices, and outside agencies, and maintaining the CONNECT System for State reporting.

How to Succeed in this Role:
- Excellent communication, problem-solving, and coordination skills
- Have an Associate’s Degree in Social/Behavioral science (required) or Bachelors in Human Services or related field of stud (preferred)
- 6 to 12 months of experience working with families of children with special needs, young children, infants, and toddlers.
- Experience working with culturally diverse families
- Passion and ability to work with any age group, birth and above

Position Summary:
The Early Development Network Service Coordinator is responsible for the identification, referral, and coordination of services for families with infants and toddlers with special needs. Provides communication with new clients, physician offices, outside agencies, and maintaining the CONNECT System for State reporting. Serves under the rules and regulations of all applicable federal, state and local laws, Department of Education, Department of Health and Human Services, and local school districts, as well as well as the philosophy, goals and objectives of ruralMED Management Resources.
Qualifications:
Education and/or Experience:
• Knowledge of family systems model; community resources; empowerment theory; family centered services; interagency collaboration: affirming minority families and their cultural diversity at a level normally acquired through any continuation of training and/or experience.
• Associates Degree in social/behavioral sciences is required, with preference for a Bachelors in Human Service or related field of study. Equivalent experience may be considered in lieu of required coursework/training.
• Approximately 6 (six) to 12 (twelve) months experience working with families of children with special needs, young children, infants and toddlers; experience with a wide variety of community agencies that provide services to young children with disabilities and families; experience working with culturally diverse families, requires the ability to work with any age group, birth and above.
Licenses/Certifications Required:
Must have and maintain at all times, a valid Nebraska driver’s license and a good driving record.
Must be able to utilize own transportation during work time.

- Serves as a point of contact with DHHS and the Department of Education for all new referrals.
- Gathers and enters all concerns and demographic information into the CONNECT System.
- Coordinates referrals with the school district to ensure the concern is being addressed and handled appropriately.
- Secures the authorizations to release information form and facilitates sharing of information.
- Notifies the referral source and other contacts as requested by the family via progress letter.
- Appropriately and confidentially uses and discloses patient protected health information only as it applies to job functions, in amounts minimally necessary for intended purpose.
- Follow procedures for coordination of CAPTA referrals with DHHS case worker.
- Follow up on medical records and authorizations for ACCESS Nebraska.
- Compiles CONNECT reports for Planning Regions Team (PRT), school district and services coordination meeting’s to assure continuous quality improvement activities with the overall goal of identifying gaps and barriers in the community.
- Makes referrals to appropriate healthcare professionals, hospital and community resources as needed for the family.
- Maintains database for all Early Development Network clients.
- Communicates in a positive, professional, and effective manner, with clients, physician’s offices, outside agencies, to ensure timely referral of intake application and that State mandated timelines are adhered to.
- Participates in staff meetings, training programs, workshops and conferences as required.
- Abides by the requirements of all policy and procedure manuals.
